Patient Handbook
Welcome to Capital Cardiology AssociatesWelcome to Capital Cardiology Associates!
Thank you for trusting us with your care. We are committed to improving your quality of life! Since opening our doors as a benchmark cardiology practice over 20 years ago, we have continued to build on our reputation as the Capital Region’s leading cardiology practice in the treatment of cardiovascular disease.
Upon request, new patients are given a printed copy of our Patient Handbook. For patients who would like an electronic version, major portions of the handbook are repeated below. Capital Cardiology Associates also offers several “paper-free” options that reduce waste and save time accessing your records, communicating with your doctor, or paying your bill.
We understand that as a new patient, you, your family members and friends will most likely have many questions on your condition, procedure, and treatment. By using the links on this page, you will be able to download, print, and complete forms our team will request from you during your first visit. We have also provided extensive information on a majority of cardiac health areas. Our Patient Education portal includes videos that help explain complex medical topics. Please keep track of any and all questions you have to share with your doctor during your next appointment.
Our Patient Handbook was created by our board certified cardiologists and caring and competent staff. We offer the highest quality of care in a comfortable, supportive atmosphere in all of our offices and locations. If you have any questions about our services, policies or other information, please don’t hesitate to ask one of team members.
Thank you again for choosing Capital Cardiology Associates for your medical needs.

Clinical Research
At Capital Cardiology Associates, with the help of our research staff and patient volunteers, we focus on finding the best treatments for people with cardiac illness. We conduct Phase 3 clinical trials, the last step in the trial process before a treatment is approved for the market. Clinical trials are designed to ensure that new treatments created to improve health are safe and effective. If you are interested in participating in a Clinical Trial, please let your cardiologist know.

Interventional Procedures
The TAVR procedure (sometimes referred to as TAVI – Transcatheter Aortic Valve Implant) is an alternative to traditional open-heart surgery. TAVR not only offers an improved quality of life for the patient, but is an alternative for high-risk patients with severe artery disease who may not otherwise be a candidate for valve replacement.

Patient Responsibility
Effective January 1, 2017 Capital Cardiology Associates will be charging a $40.00 fee for a missed appointment. If you call and reschedule or cancel your appointment 24 hours prior to your scheduled appointment, a fee will not be charged. Please understand that this fee is not covered by your insurance plan. In order to cancel or reschedule an appointment patients need to call 1-518-292-6000.
By providing CCA with at least 24 hours prior notice of any cancellations it will free up time to see patients who need immediate appointments which happens frequently in a cardiac practice.
Our purpose is not to burden you with unnecessary out of pocket expense but rather encourage you to be considerate to others throughout our community who may be in urgent need of our services.
Thank you for understanding and allowing us to provide easily accessible service to the patients in our practice. We look forward to seeing you at your next visit.
